Full job description

Coordinates, tracks, and implements outreach to patients and families on behalf of community providers that serve PFK members. Supports the work of their peers in the Population Health Partnerships department related to relationship building and quality improvement.

  • Communicates with primary care physicians, specialty physicians, behavioral health physicians, community provider office staff, and other service providers to identify and implement outreach services.
  • Reports out monthly on the impact of outreach services utilizing tools created by the PFK data team, claims data, Epic, and data pulled from the community provider EMR.
  • Identifies innovative outreach opportunities; collaborate with PFK team to design and pilot new outreach methods.
  • Collaborates with PFK Quality Improvement, Pharmacy Team, and Provider Relations on outreach efforts and other support opportunities associated with their project work.
  • Serves as a liaison with NCH marketing to design and order outreach materials. Develops and maintains a standard collection of outreach materials.
  • Develops tracking resources to support member identification and outreach impact.

Bachelor’s Degree in health care, public health, or related clinical field, preferred.

Valid Ohio driver’s license and proof of auto insurance as required by hospital policy and position-specific requirements. Must pass motor vehicle background inspection, insurance eligibility, driving qualifications, and training set forth by Nationwide Children’s Hospital and maintain qualification of insurance guidelines.

Working knowledge of Medicaid and other regulatory agency standards.

Experience working in healthcare in a patient facing role, preferred.
