Physical Therapy Assistant, PTA - Home Health - PRN CORE

What You'll Do Deliver High-Quality Patient Care - Provide direct physical therapy services under the direction of a licensed Physical Therapist - Follow individualized plans of care to help patients reduce pain, improve mobility, and restore function - Assist patients with therapeutic exercises, gait training, balance, and functional mobility - Support patients with transfers, strengthening, and activities of daily living - Utilize therapeutic modalities such as heat, cold, ultrasound, and electrical stimulation as appropriate - Educate patients and caregivers on exercises, safety, and use of assistive devices Monitor Progress & Communicate - Observe and report patient progress, response to treatment, and any changes in condition - Maintain accurate, timely documentation in the EMR - Communicate effectively with the Physical Therapist and interdisciplinary care team - Participate in case conferences and care coordination efforts Collaborate & Contribute - Work closely with nurses, therapists, and other healthcare professionals to ensure coordinated care - Participate in team meetings, in-services, and ongoing training - Uphold clinical standards and contribute to quality improvement efforts What Sets You Apart - Strong patient-first mindset with a passion for rehabilitation - Ability to motivate and engage patients throughout recovery - Excellent communication and time management skills - A compassionate, proactive approach aligned with our values: Trustworthy.
