Education: High school
Key Responsibilities Assist residents and their families with the facility admission and orientation process. Help coordinate safe, comprehensive discharge planning and community resource referrals. Maintain accurate, up-to-date resident medical records and social service documentation.
Conduct regular room visits to assess residents' emotional and psychosocial well-being. Advocate for resident rights and promptly report any grievances to the Director. Assist in organizing resident council meetings and family support initiatives.
Qualifications High school diploma required; Associate or Bachelor's degree in Social Work or Psychology preferred. Prior experience in a skilled nursing facility or long-term care setting is highly desirable. Strong written and verbal communication skills in English; bilingual skills are a major plus.
High level of empathy, patience, and professionalism when interacting with elderly residents. Proficiency with basic computer programs and electronic health records (EHR) systems. We are an equal opportunity employer.
