Bilingual RN Care Coach - Bonus Eligible

<p><strong>This is a remote role.</strong> CircleLink Health is looking for passionate, tech savvy registered nurses to work remotely and serve patients enrolled in Medicare’s Chronic Care Management Program. In this <strong>part time role</strong> (about 20-25 hrs. per week), an RN Care Coach will be assigned a group of patients that they will be following and calling each month.

In these monthly calls you will provide education, coordinate care, close preventive care gaps, and coach on strategies for self-management to keep them out of the hospital.</p><p>This Role Requires Precision, Discipline, and Accountability</p><p>The Care Manager role is not a step back from bedside nursing — it’s a step into a more complex, structured, and performance-driven environment.

Responsibilities:</p><ul> <li>Utilize our specialized care management software to call Medicare patients with 2 or more chronic conditions (Diabetes, CHF, Chronic Pain, COPD, etc.) on a monthly basis</li> <li>Build and maintain rapport with patients to help coach them to improved health through SMART goals and education on self-management strategies</li> <li>Implement and improve the Plan of Care by updating medications, appointments due, biometrics, symptoms, and interventions made</li> <li>Connect the patient with community resources as needed, including transportation, personal care needs, prescription/DME assistance, social services, etc.</li> <li>Conduct Transitional Care Management activities to high risk patients discharged from the hospital and the ER to reduce unnecessary readmissions.</li> <li>Close care gaps by encouraging and assisting with preventive care measures, i.e. annual well visits, vaccines, cancer screens, follow-up/specialist appointments, etc.</li> </ul><p><strong>Requirements</strong></p><ul><li>Fluent in English and Spanish is a <strong>must</strong> </li><li>Self-directed, able to work independently with little supervision while meeting performance metrics</li><li>Passion for nursing and improving patient outcomes</li><li>Good with technology and eager to learn and use new software</li><li>Excellent organizational and time management skills</li><li>Timely communication is essential, and nurses are expected to respond to all messages and emails within <strong>24–48 </strong>hours. </li><li>Strong critical thinking and problem-solving skills</li></ul><p>Education and Experience:</p><ul><li><a href="https://www.ncsbn.org/nurse-licensure-compact.htm" rel="nofollow noreferrer noopener" class="external"></a>Current, unrestricted Multistate RN<a href="https://www.ncsbn.org/nurse-licensure-compact.htm" rel="nofollow noreferrer noopener" class="external"></a> <strong>or</strong> New Mexico RN license is required.

Requirements:</p><ul><li>RN needs a STRONG internet-connected computer.</li><li>You must have a minimum of <strong>20 hours of availability</strong> per week sometime between the hours of 8 am and 5 pm <strong>MST,</strong> Monday- Saturday. You do not have to work every day-your schedule is flexible.</li><li>This is a 1099 contract position with no end date.

Care coaches are responsible for their own equipment, taxes and insurance.</li></ul><p><strong>Benefits</strong></p><p><strong>

Compensation:</strong></p><p>RN Care Coach compensation is paid at the rate of $15.00 for the first 20-minute increment (20-min milestone), $12 for the second 20-minute increment (40-min milestone), and$11.50 for the 3rd 20-minute increment (60-min milestone).</p><p>A&nbsp;<em>clinical encounter&nbsp;</em>occurs after two criteria are met:1/ A successful interactive&nbsp;<em>clinical call</em>&nbsp;with the patient or their caregiver.2/ A 20-min, or 40-min, or 60-min milestone is met on the patient's chart.

Example pay scenarios, but not limited to:</p><ul><li>Two 20-min successful calls with 2&nbsp;<em>different</em>&nbsp;patients pays $30 for the two 20-min visits. ($15+$15=$30)</li></ul><ul><li>A 40-min encounter with the <em>same </em>patient pays $27. ($15+$12=$27).</li></ul><ul><li>A 60-min encounter with the <em>same</em> patient pays $38.50. ($15+$12+$11.50=$38.50)</li></ul><p><strong>Bonus Eligibility:</strong> Coaches who successfully complete <strong>THREE</strong> months of employment and meet <strong>all </strong>performance expectations will be eligible for a $300 performance bonus.</p><p><strong>Pay Timing:</strong></p><p>Monthly via direct deposit, 40 days after the last day of the first month of service.
