Our platform automates charting and compliance so you can focus on high-quality senior care.</p><p>Role Overview<br> We’re seeking Primary Care NPs to provide on-site care to Assisted Living/RCFE residents across Stockton, Modesto, and Amador.</p><p>What You’ll Do</p><ul><li>Routine &amp; urgent assessments; chronic disease management</li><li>Post-discharge follow-ups, med reconciliation, labs/diagnostics</li><li>Coordinate with facility staff, specialists, families/POAs</li><li>Same-day documentation; CMS/regulatory compliance</li></ul><p>Schedule &amp; Setting<br>Part-time position with a structured weekday schedule.
