Full job description

Essential Duties Interview patients to determine medical problems / condition, and document in chart for physician. Obtain and record patient's vital signs and weight. Complete daily charting, monthly summaries, comprehensive notes, wound charting, restraint documentation and auditing of monthly orders.

Collect samples, such as urine, blood, and sputum, from patients for testing, and perform routine laboratory tests on samples. Record food and fluid intake and output. Document services performed for billing purposes.

Dress wounds and give enemas, douches, alcohol rubs, and massages. Assemble and use equipment such as catheters, tracheotomy tubes, and oxygen suppliers. Apply compresses, ice bags, and hot water bottles.

Prepare or examine food trays for prescribed diets and feed patients. Monitor and report changes in physical and mental status, vital signs, and assess needs for planned care. Observe patients and report adverse reactions to medication or treatment to medical personnel in charge.

Administer specified medication orally or by subcutaneous or inter-muscular injection, and note time and amount on patients' charts. Review physician's orders, lab requests, or follow-up needs with patient. Provide direct patient care by assisting with feeding, toileting, bathing and dressing.

Provide assistance and one-on-one interaction with needy residents. Assist patients with walking and turning. Communicate residents' status and/or needs to the supervisor or charge nurse, MD, consultants and Nurses Assistants.

Supervise activities by directing, communicating and monitoring Certified Nurse Assistants (CNA's). Perform other duties, tasks and special projects, as required.

Qualifications/Preferred Skills, Knowledge & Experience High School Diploma or General Education Degree (GED). LPN license. Current Certification in CPR.

Six months to one year of job-related experience and/or training. Vocational training as an LPN. Mandated in-services to meet state regulations.

Educational and clinical experience required by State Nursing Board. Ability to establish priorities and remain focused on daily operations. Effective interpersonal and customer service skills.

Strong organizational, time management, and documentation skills. Accurate and detail oriented with strong oral and written communication skills. Ability to work independently or as part of a team and proceed with objectives.

Ability to follow protocols to handle and resolve recurring problems. Ability to use common sense understanding to carry out written and verbal instructions. Flexible, with an ability to carry multiple tasks and projects to completion Exhibits a strong work ethic and a willingness to learn new tasks.

Professionalism, integrity, and honesty with an ability to handle confidential matters. Preferred Skills, Knowledge & Experience: One or more years of job-related expereience. Attend in-services and seminars related to geriatric nursing.

Strong knowledge of Pennsylvania state regulations. Ability to complete continuing education and remain updated on health care field via classes, seminars, magazines, journals, literature and videos. Knowledge of Pocopson Home policies and procedures.

Working knowledge of Nursing Board regulations. Additional Information Relevant background checks, clearances, physicals, drug tests, and any other assessments or evaluations will be conducted after an offer of employment is given per County policy. Physical Demands: While performing the duties of this position, the employee is frequently required to stand; walk; reach above shoulders and work with arms above shoulder height; bend at the waist or work bent at the waist; climb stairs; push wheelchairs or other items; lift patients and items; work with machinery (e.g., feeding pumps, oxygen tanks); talk or hear; and taste or smell.

Occasionally, you will need to sit; kneel, stoop, crouch or squat; carry items; and work with an uncommon level of noise. On rare occasions, you will need to crawl, and twist or rotate at the waist. There are no specific vision requirements listed for this position.
