The RN Minimum Data Set (MDS) Supervisor is responsible for the accurate and timely completion and transmission of MDS assessments for all residents in facility. This role oversees the MDS nurses/coordinators to ensure residents are properly assessed and that this information is accurately documented in their electronic health records (EHR). The Supervisor participates as an active member of the interdisciplinary team and leads care plan meetings. Collaborates with the DON and Director to provide information on facility residents, including audits, areas of opportunity for care improvement and care conferences, as well as quality measures.
Education
• High School Diploma, High School Equivalency (HSE) or Completion of a CHS Approved Individualized Education Plan (IEP) Certificate required
• Completion of MDS Assessment training class required
• Bachelor's Degree in Nursing preferred
Experience
• 3 years of long term care experience required
• Experience in completion of MDS assessments required
Licenses and Certifications
• RN - Current and valid Registered Nurse license to work within the state of California required
• BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
