What You'll Do... - Work in the community performing home visits. - Ensure consumer involvement in the Person-Centered Planning Process (PCP). - Facilitate the implementation of Individual Plan of Service. - Perform comprehensive, ongoing psychosocial assessments. - Participate in legal proceedings including petitioning and testifying in clinical emergencies. - Provide community-based services. - Facilitate the coordination of consumer intakes, discharges, and transfers. - Communicate with all service providers in Treatment Plan. - Facilitate hospitalizations, attend hospital meetings and coordinate with hospital social workers. - Coordinate and monitor activities such as a job, school, volunteerism, and family caretaking. - Provide referral to appropriate community resources.

Required: - Bachelor's degree in a human services field from an accredited institution. - Valid Michigan Driver's License. - Willingness to work in the community and perform home visits. Preferred: - LLBSW, LBSW, LLMSW, LMSW, LMFT - Prior case management experience in a mental health or rehabilitation setting preferred.
