Full job description

Hourly Pay Range: $21.08 - $31.62 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.

- Location: Chicago, IL (with the ability to travel to other locations locally)

- Hours: Monday-Friday, 8:00a-4:30p M-F with evening and Saturday availability as needed.

A frontline public health worker who is a trusted member of and/or has a deep connection or understanding of the community served. This trusting relationship enables the CHW to serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery.

- Assess and address patient's needs in the area of housing, food insecurity, transportation or other non-medical areas that influence a patient's health outcomes (Social Determinants of Health).

- In partnership with patient care teams, provides support for patients in gaining access to medical services, screenings, prescription medication and durable medical equipment.

- Assist patients with getting access to social services and other community-based resources.

- Follows up with patients via phone calls, home visits and visits to other settings where patients can meet.

- Maintain accurate documentation of patient contacts and document patient's needs, action plan and outcomes in the medical record system.

- Participate in continuing education, trainings and CHW organizations.

- Community Health Worker, Health Coach, Patient Advocate Certification and/or equivalent preferred

- Strong passion, leadership and interpersonal skills necessary to effectively work with multi-cultural, diverse populations.

- Ability to effectively engage in conversations, motivate and encourage patients to take steps to maintain and improve their health.

- Creative thinker with ability to problem solve and brainstorm solutions.

- Ability to work collaboratively as a part of a patient care team.

- Basic computer skills required. Experience with EPIC EMR preferred.

- Fluency in Russian, Spanish, Urdu, Hindi, or Arabic a plus.

- Premium pay such as shift, on call, holiday and more based on an employee's job (For eligible positions)
