Full job description

The Case Aide is responsible for assisting families to meet concrete needs to increase safety, reduce risk and promote wellbeing utilizing the Family Treatment and Rehabilitation (FTR) model of practice. The Case Aide works closely with a team of Case Planners and Supervisor to insure all responsibilities are carried out in accordance with model fidelity and the standards of FTR, ACS and SCO Family of Services.

  • Work collaboratively with a team of Case Planners and Supervisor to provide assessment, goal setting, interventions and case management to prevent foster care and promote wellbeing.
  • Conduct home and office visits and accompany clients to appointments with housing, public benefits, court and other service providers.
  • Provide outreach, referrals, escort and advocacy to connect families with resources, increase economic stability and prevent eviction.
  • Assist families to complete applications and documentation as needed.
  • Request collateral documentation from schools, medical providers and other service providers.
  • Support team to assess safety and risk and assist to implement crisis management plans and safety plans as needed.
  • Develop positive collaborative relationships with community based resources on behalf of program.
  • Assist to implement groups and social activities for clients including providing child care during groups as needed.
  • Complete all required documentation and data entry within deadlines.
  • Attend individual and group supervision and all necessary meetings and trainings.
  • Provide administrative support to office as needed: reception, filing, and other responsibilities as assigned.
  • Demonstrate strong sensitivity to cultural and socioeconomic diversity.
  • Commit to being an active contributor to the vision and mission of the program.
  • Work to create and support a respectful learning environment.
