Full job description

What You'll Be Doing: - Serve as a role model and resource for nursing staff and interdisciplinary team members - Support staff in problem-solving and determining appropriate clinical interventions - Collaborate with the Administrator on hiring, orientation, retention, and performance management - Adhere to budget requirements for labor, pharmacy, DME, and medical supplies - Coordinate patient care with RN Case Managers and attending physicians - Review and manage patients receiving higher levels of care (respite, inpatient, continuous care) - Oversee clinical and management responsibilities for assigned teams - Serve as liaison between Administrator, clinical staff, patients, families, and community partners

Qualifications: - Graduate of an accredited school of nursing with a bachelor's degree or higher in nursing or a related health field. - Current, unencumbered Registered Nurse (RN) license in the state of employment. - Active CPR certification required; certification in Hospice and Palliative Nursing preferred. - Must meet one of the following: - Bachelor's degree or higher in nursing or a related health field and at least 2 years of full-time supervisory or managerial experience in a hospice or home health agency within the last 5 years - At least 4 years of full-time supervisory or managerial experience in a hospice or home health agency within the last 5 years - Experience with Electronic Medical Records (EMR) preferred; familiarity with WellSky is highly desirable. - Valid driver's license with a clean driving record. - Reliable transportation and willingness to travel as needed. - Ability to lead bereavement support groups and make sound clinical decisions. - Flexibility to work outside regular hours based on patient needs.
