RN Home Health Case Manager

Are you an experienced RN, Home Health Case Manager in the Columbus, OH area? Are you looking for a new opportunity with a prestigious healthcare company? Then this is the position for you for you!

Home care nurses play a pivotal role in the healthcare field by preventing hospitalizations and assisting clients to remain independent in their homes. Requires a high level of independence, sound clinical judgment, excellent clinical skills and the ability to work with and communicate with a range of age groups. Typical clients include pre-term infants with special needs, individuals who are post-surgical and require follow-up care such as antibiotic therapy or wound care; those with chronic conditions (CHF, diabetes) in need of additional education, laboratory work or therapy; and individuals who require long-term support for medication management.

Our nurses provide clients with education, treatment, and emotional support to help individuals recover from injuries and illnesses. Good communication skills, the ability to coordinate services, make referrals to other agencies/providers, and function as a client advocate are necessary. In general a full-time RN will have a caseload of 12-15 clients.

A reliable vehicle is necessary for travel in a 50 mile radius of Columbus.

Territory: Far North of Delaware, far South of Lancaster, far West of London, far East as Newark
