What You'll Do Care Coordination & Service Facilitation - Conduct functional assessments and re-assessments. - Develop individualized service plans in partnership with clients, families, support systems, and providers. - Coordinate and authorize services as appropriate. - Facilitate recovery-focused and family-centered team meetings. - Connect clients to community resources and support services.
Documentation & Collaboration - - Maintain accurate client records and documentation. - Complete required reports and service documentation on time. - Collaborate with treatment teams, community providers, schools, healthcare professionals, and natural supports. - Participate in supervision, training, and staff development opportunities.
What We're Looking For Required
Qualifications - Master's degree in a human services-related field. - Valid driver's license and reliable transportation. - Ability to travel throughout Dane County to meet clients in community settings. - Ability to work flexible hours, including occasional evenings based on client needs.
Preferred
Qualifications - Experience providing mental health, behavioral health, therapy, case management, care coordination, or community-based services. - Experience working with children, families, and adults with complex needs. - Experience in recovery-oriented, trauma-informed, or person-centered service delivery.
