On-Call Social Workers (Per Diem) (Brooklyn/Queens/Nassau and Manhattan)

The Hospice Social Worker provides medical social services, psychosocial and bereavement counseling to the hospice Patients and families.

- Minimum one (1) year bereavement counseling experience is preferred. - At least one year of Hospice or Home Care experience preferred. - Excellent interpersonal, and communication skills. - Position requires ability to commute to multiple territories to ensure continuity of care. - A valid driver's license is required. - A vehicle is required to commute to patients. (This requirement does not apply to positions assigned exclusively to Manhattan territory) - Exhibits an ability and attitude relative to the spirit of caring and tradition at Calvary Hospital as required by specific job duties and functions.

- MSW from an accredited school of social work and one year related experience. - New York LMSW or LCSW eligible.
